二、申请人的资格要求：



1.满足《中华人民共和国政府采购法》第二十二条规定;

合同包1(全自动荧光核型滴度判读系统)特定资格要求如下:





(1)（1）提供参与本标段投标供应商有效的医疗器械经营许可证； （2）提供参与本标段投标供应商有效的食品药品经营许可证，经营范围包括三类医疗器械； （3）提供参与本标段投标供应商有效的医疗器械生产许可证； 以上（1）—（3）项任意提供一项即可。







(2)若所投产品为进口产品，则生产厂家直接参与投标的须提供生产资格证明文件。经销商参与投标的需提供所投产品生产厂家或中国总代理商或区域代理商出具的长期经销代理权或长期授权或针对本项目的授权书。中国总代理商或区域代理商出具长期经销代理权或长期授权或针对本项目的授权书的，需提供中国总代理商或区域代理商与生产厂家的关系证明材料。







合同包2(前庭康复训练仪)特定资格要求如下:
